Waiver of Liability - Shadow House Co., Inc.

Agree to Release and Hold Harmless *

By signing this agreement, I hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Shadow House Co., Inc. and its employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death or otherwise, regardless of whether injuries are caused by negligence. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally and will ask for clarification when needed.

Acknowledgment of Risks *

I fully understand and acknowledge that the activities at The Shadow House, involve inherent risks, dangers and hazards which may result in injury, illness or death. These include but are not limited to the use of power equipment including table saws, routers, bandsaws and drills. I swear that I am participating voluntarily and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

Governing Actions *

I acknowledge that this Accident Waiver and Release of Liability Form will be used by Shadow House, and that it will govern my actions and responsibilities during the use of the workshops.

In consideration of my use of the workshop(s), I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: *

(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: The Shadow House and its employees;
(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.

Medical Treatment *

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.

Surveillance *

I also acknowledge that during my use of the workshop(s), surveillance video may be taken.

The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. *

By Signing below I forfeit any and all right to bring a suit against The Shadow House Co, Inc. and its employees for any reason. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
